The following Procedures are processes associated with Policies.

Policies are the "what" and Procedures are the "how". The LCPOA Board approves the Policies, with some Policies requiring Procedures be developed to implement the Policies.

If a form has "fillable" fields, it simply allows you the option of filling in the blanks online if you choose. Please note that your information will not be saved, so you will need to print it out before closing the form.

Please return forms to the LCPOA office by mail, fax, drop it off at the office, or scan your filled out form and send it to us by email.
