2012 Payment of Dues - Option 1 |
TO MAIL A CHECK TO CAB: Tear off the coupon at the bottom of the invoice and insert it in the window envelope provided along with a completed check. Allow 4-5 days from when the check is mailed to when payment will be posted to your account at the Association office.

2012 Payment of Dues - Option 2 |
CAB ONLINE E-CHECK: Members may access the CAB web site (http://www.mutualofomahabank.com/west/association_banking/) and click on “By E-Check” under “Homeowner’s Pay Assessments”. Online payments are posted the next day to the member’s LCPOA account. Use the following information to complete the online form:
1. Mgmt. Co. ID: 6084
2. Assoc. ID: LAKE
3. Association Name: Lake Columbia POA
4. Management Co.: Lake Columbia POA
5. Association Account #: (see online sample and use information from the 16 character segment on the scan line on the coupon (e.g. 000000000005468H). See FAQ # 18 for more information.
6. Payment Amount: (Total Due as shown on the invoice)
7. Checking Account # (obtained from the check – see online sample)
8. Routing #: (obtained from the check – see online sample)

CAB ONLINE CREDIT CARD PAYMENTS: Members may access the CAB web site (www.mutualofomahabank.com/west/association_banking/) and click on “By Credit Card” under “Homeowners Pay Assessments”. Complete the online form. Only Master Card, Discover, Novus and American Express are accepted. There will be an additional convenience fee of $15 for amounts of $150.01 - $500 and $25 for amounts greater than $500.00 added to the charge by CAB. Use the following information to complete the online form:
1. Mgmt. Co ID: 6084
2. Assoc. ID: LAKE
3. Association Name: Lake Columbia POA
4. Mgmt. Company: Lake Columbia POA

ONLINE BILL PAY: Members may set up online bill pay through their personal bank account if it is offered. A hard copy check will be sent to CAB. When completing the online form, the Name of Account should be Lake Columbia POA, the Account # is the 16 character segment of the scan line on the coupon (example: 000000000005468H), and the CAB mailing address is: Community Association Banc, P. O. Box 65705, Lake Columbia POA, Phoenix AZ 85082-5705.

CHECK SCAN AT LCPOA OFFICE: Members may bring their check and coupon to the office and have it scanned by equipment provided by CAB. This will result in an immediate debit to the member’s checking account and payment will be posted the following day to the member’s account at the LCPOA office.

CREDIT CARD PAYMENT AT LCPOA OFFICE: As in the past, members may use charge cards to pay their dues. On the reverse side of the coupon is a form to enter the card number, expiration date, cardholder address, phone number and signature line. This form may be submitted in person at the Office or mailed or faxed to the Office. Only Visa, Master Card and Discover cards are accepted. DO NOT SUBMIT THIS FORM TO CAB – ONLY TO LCPOA.

Yard Waste Pick-Up Begins (5/5) - Every Saturday |
Yard waste before 5/1 is 50¢ per bag. Call Northwest at 787-3699 for pick up arrangements. After 5/5 it is part of the regular trash service and is every Sat. |
